  • This includes: Nurses, Doctors, Dentists, and Chiropractors, and other healthcare workers that have direct contact with patients.

  • To schedule an appointment, you must fill out the Consent Form.
  • Once you have completed the Consent Form, you will be directed to the Appointment Scheduler.
  • Please select a date and time for your appointment, and be sure to include your e-mail address.
  • You will then be sent a confirmation e-mail with the information you have selected.
